Requirements and Disclaimers

We at Corporate Longevity, a branch of the Alternative Pain Relief Center in Escondido, are committed to customer service and performance excellence. Our therapists are highly skilled and have great success in the elimination and management of chronic degenerative disease derived pain symptoms. Even though our success rate is very high, we cannot claim to heal anyone and we are no doctors, so each employee has the duty to disclose all medical conditions and might be asked to obtain medical clearance from their primary care physician in writing before a treatment. All medical information will be kept in a digital vault that is HIPAA approved. All medical intake forms are for our therapists eyes only to determine the most appropriate therapy and to expose any possible contraindications. Medical intake forms will under no circumstances be shared with employers and kept confidential. Alternative Pain Relief Center Escondido, LLC DBH Corporate Longevity does not promise any healing and does not guarantee any improvement in pain conditions. Some conditions fall out of our scope of practice or are too advanced to be reversed. Patients can always have underlying conditions not known to the patient or therapist that can lead to complications. Corporate Longevity can only determine the safety of a treatment when the client divulges all medical conditions. If a client refuses to divulge medical information, we cannot treat that client for safety reasons. Medical massage and alternative pain relief treatments are not comparable to a relaxing massage. Discomfort or bruising are common side effects of medical massage and each employee is asked to sign, acknowledge and agree to these terms.

REQUIREMENTS

The employer has to provide a private room for massages that is big enough for a massage table, space around it and a table for massage tools and instruments.

Besides formal corporate contracts, each client needs to fill out several forms, waivers and intake protocols in order to receive treatment. 

Each employer is responsible for their own "hold harmless" agreements with their employees.

Each session is video recorded for training and liability purposes. Corporate Longevity has strict harassment regulations and our therapists can stop each session at any time if they are not feeling comfortable. If the therapist is forced to end a session because of an employees conduct, the session will not be refunded.

Each employee is asked to bring tight fitting gym clothes. The clothes need to be tight shorts and tight short sleeved t-shirt for male or female. The room need to be climate controlled with a temperature of 70-72 degrees. We do not work on clients in their office attires. Each client is asked to shower in the morning prior to their massage session and come well groomed.
